Position reports to the Director of Clinical Outcomes. Work with multi-disciplinary care teams managing the clinical outcomes process for the entire patient population. Provide leadership and facilitate the application of evidence based practices clinical improvement efforts and current benchmark data to targeted patient populations.
Collect submit and analyze clinical data from internal and external data bases developing and presenting reports leading performance improvement efforts lead multi-disciplinary teams with proven results.
Review patient population case loads on a daily bases assessing and intervening with medical staff to assure timely implementation of best practices and quality initiatives supporting and providing educational offerings for staff.
Requirements: 2 years quality management experience, strong computer competency experience required to include MS Office (Word Excel Outlook Power Point) as well as trending and data analysis software tools.
• EDUCATION: Bachelors or Masters Degree in a healthcare related field (Nursing Microbiology Public Health etc.) Registered Nurse (RN) license (or eligible) a plus CPHQ preferred
