Work in conjunction with a multi-disciplinary care team to manage the clinical outcomes process for the entire patient population.
Provide leadership and facilitate the application of evidence based practices clinical improvement efforts and current benchmark data to targeted patient populations.
The Clinical Outcomes Coordinator will be responsible for the collection submission and analysis of clinical data from internal and external data bases; developing and presenting reports; will lead performance improvement efforts; lead multi-disciplinary teams with proven results; review patient population case loads on a daily basis.
Assess and intervene with medical staff to assure timely implementation of best practices and quality initiatives; support and provide educational offerings for staff. Position reports to the Director of Clinical Outcomes.
Requirements: Bachelors or Masters Degree in a healthcare related field (Nursing Microbiology Public Health etc.); Texas Registered Nurse (RN) licensure (or eligible) a plus CPHQ preferred and/or 2 years quality management experience; as well as trending and data analysis software tools
